Redesigned medical forms covering general patient admission to verification and screening forms for diagnostic imaging. Also including Imaging Exam Prep Packet for patients before imaging tests including Mammograms, MRI, CT, Ultrasounds, Nuclear Medicine and X-Rays. Along with the packet the patient is given a color matching wristbands designated to the particular scan they are receiving. The purpose being to remind the patient of the rules or what they can and can not do before their diagnostic imaging scan.
